Three Easy Ways to Obtain a Birth or Death Record

There are three ways in which to obtain a birth or death record in Missouri.  Choose the method that best suits your needs:

1.  Walk-in – Same day service is available to walk-in customers. This is the fastest way to obtain a birth certificate. When you arrive, you will complete an application and pay the $15.00 required fee.     Full instructions>>

2.  Mail – You may download this form, complete it and send it to us with both payment and a photocopy of your driver's license. Turn around is estimated at 3 to 5 weeks from the day the request is received. However, people are urged to allow sufficient time for delivery for all birth/ death records. Mail a check or money order of $15.00 for each certificate. 

A photocopy of your driver's license must be sent with payment
or your payment will be returned to you without the certificate.

3.  VitalChek Network – Obtain a certified copy of a birth or death certificate within 7-10 business days by credit card, with Federal Express or regular mail. Additional service charges will be applied.     Full instructions>>
