Birth/ Death Certificate Walk-In

Birth & Death Certificates are available at the Health Center from 8:00 AM- 4:30 PM weekdays.  The fee is $15 for certified birth certificates.  Certified copies of a death record ordered at the same time for the same person are $13 for first copy and $10 for each additional copy per person per visit. 

If an applicant requests two copies of a death record in the morning, the fee would be $23.  If the applicant requests an additional two copies in the afternoon, it is considered a second request and the fee would be $23.  If an applicant requests two copies of a death record for one person ($23) and one copy of a death record for another person ($13), the fee would be $36.  Local public health agencies will continue to keep all Vital Records fees collected locally. No birth certificates before 1920 and no death certificates before 1980 are available at the Health Center.  They may be obtained from Missouri Department of Health, Bureau of Vital Records, 9930 Wildwood, PO Box 570, Jefferson City, MO 65102, or, you may contact them by phone at:
