Prenatal Case Management targets pregnant women who are on Medicaid or eligible for Medicaid. The client must also be at risk as determined through an evaluation risk assessment. At risk is used to describe those who have an increased probability of an unfavorable physical, psychosocial, development and/or environmental outcome. Prenatal Case Management services will assist individuals eligible under the plan in gaining access to needed medical, social, educational and other services. The goal of Prenatal Case Management is to reduce infant mortality and low birth weight and inadequate prenatal care rates. Each client is followed by one of our registered nurse case managers through her pregnancy and delivery.
