Please note: you cannot apply for Medicaid or Medicare at the Clay County Public Health Center. Please refer to the information below for where you can apply and how to receive assistance with the process.

Medicaid

 An individual may apply for medical assistance at the County Family Support Division (FSD) Office in which he/she resides, or in some cases at outreach sites. Applications for MO HealthNet Managed Care may be made by phone by calling (888) 275-5908 or by mailing a completed MO HealthNet Managed Care application to a MO HealthNet Managed Care Service Center.

If the individual is determined eligible, the individual or family receives an approval letter from the FSD county office that is valid until they are issued a MO HealthNet identification card with the next regular payroll. Participants eligible for MO HealthNet Managed Care must enroll in a health plan. The MO HealthNet Managed Care participant must present the MO HealthNet card and the MO HealthNet Managed Care health plan card each time a medical service is rendered.

For Enrollment Assistance:
The Clay County Public Health Center offers on-site Medicaid assistance two days a week. On Tuesdays, you can make an appointment with for a full consultation by calling 816-889-1915 or emailing Markina Durham at mdurham@rodgershealth.org. On Thursdays, a representative is available to answer questions during walk-in appointments from 8 a.m. to 4:30 p.m.

Excelsior Springs Hospital also offers Medicaid enrollment assistance at Good Samaritan Center on the second Monday of every month from 1:30-2:30 p.m.

Medicare

Missouri SHIP is the Missouri State Health Insurance Assistance Program that has a highly-trained team of volunteers who offer free, unbiased assistance to help individuals looking for help navigating the Medicare program.

CCPHC has a partnership with Missouri SHIP which allows you to make an appointment with them by calling 1-800 390-3330 or clicking the MO SHIP button below.  We encourage everyone to review their Medicare plan information annually, especially during open enrollment October 15th – December 15th.
