You can make an appointment with Clay County Public Health Center by calling 816-595-4355. August is our busiest month, due to the beginning of the school year, so please schedule an appointment as soon as possible to ensure availability.
Exemptions
Clay County Public Health Center strongly encourages parents to immunize their children. Vaccines are safe, effective, and our best protection against dangerous and sometimes deadly diseases.
Religious (Imm.P.11A) and Medical (Imm.P.12) exemptions are allowed in Missouri K-12 schools. Parent/Guardian (ImmP.11) exemptions and medical exemptions are allowed in Missouri childcare facilities. The appropriate exemption card must be on file with your school district or childcare facility. Unimmunized children are subject to exclusion from childcare/preschool/school when outbreaks of vaccine-preventable diseases occur.
Claiming a religious exemption indicates a parent or guardian’s sincere belief that the family’s religious preference does not support immunizing against vaccine-preventable diseases. It is a parent’s/guardian’s right to actively choose not to immunize their child by claiming a religious exemption or parental exemption, but this decision carries risks that can significantly impact their child, their family, their classmates, and the community.
A medical exemption is authorized upon signed certification by a licensed Doctor of Medicine (MD, Doctor of Osteopathy (DO), or their designate indicating that either the immunization would seriously endanger the student’s health or life, or the student has documentation of disease or laboratory evidence of immunity to the disease.
Immunization exemption forms (medical, religious, immunizations in progress, and parent/guardian for childcare/preschool) can be accessed and printed online from the Missouri Department of Health and Senior Services website on the school and childcare pages of the immunizations section.
