Clay County Public Health Center can print certified copies of original birth certificates for individuals who were born in the state of Missouri after 1920.
Death Certificates
Clay County Public Health Center can print certified copies of original death certificates for individuals who were deceased in the state of Missouri after 1980.
Update for Funeral Homes - Fall 2024
The Clay County Public Health Vital Records Program is excited to announce a new service for funeral homes that will make the process of getting death certificates much easier. Starting November 5, 2024, there will be a separate portal available for funeral home-associated clients to order certificates online through VitalChek.
We look forward to continuing our tradition of quick and quality service through this tool, processing orders up to two times per day. Directors interested in learning more about this service can call 816-595-4204 or email awelsh@clayhealth.com.
Fees
- Birth Certificates: $15 per copy
- Death Certificates: $14 for first copy, $11 for each additional copy per decedent
Get the Certificates You Need
There are multiple ways to get certificates from Clay County Public Health Center. See below for more details.
Birth and death certificates are available at the health center from 8 a.m. - 4:30 p.m. Monday to Friday, excluding holidays. Same-day service is available to walk-in customers. You must have a photo identification card of some kind, e.g. driver's license, work I.D., school I.D., etc. to obtain the copies. Click here for a full list of accepted forms of photo I.D. including alternate forms of identification.
You can fill out the application in advance: Application for Certified Copy of Birth/Death Certificate for the State of Missouri. Please print the document and bring it when you come; otherwise, the form will be provided when you arrive.
Download the Application for Certified Copy of Birth/Death Certificate for the State of Missouri. You MUST have the form notarized by an official notary public and send it to Clay County Public Health Center with a check or money for the fees listed above. Please make checks or money orders payable to "CCPHC". Turnaround time is estimated at 7-10 business days from receiving your request. However, please plan to allow sufficient time for delivery of all birth/death records.

Use the VitalChek Network to obtain a certified copy of a birth or death certificate within 7-10 business days by credit card, with Federal Express or regular mail. Additional service charges will be applied.
Clay County Public Health Center does not accept credit card orders over the phone or online orders. However, for your convenience, you can process online requests through an independent company that we have partnered with to provide you this service, VitalChek Network, Inc. An additional fee is charged by VitalChek for using this service, and all major credit cards are accepted, including American Express, Discover, MasterCard or Visa.

No birth certificates before 1920 and no death certificates before 1980 are available at the health center. For assistance, please contact the Missouri Department of Health, Bureau of Vital Records at 930 Wildwood, P.O. Box 570, Jefferson City, MO 65102.
