Immunizations Record

There are two free ways to get access to your own and your family's immunization records.

Option 1: Mobile App/Web Platform

Access immunizations records that have been reported to ShowMeVax, Missouri's statewide immunization information system, through the Docket® app. The app is available to download at no cost on the Apple App Store or Google Play Store. You can also access Docket on the web.

The app offers a few helpful features like adding all family members to one account, setting up immunization reminders, and accessing back-to-school forms.

Option 2: Clay County Public Health Center

Clay County Public Health Center can provide record of immunizations from two different sources.

• Immunizations received at Clay County Public Health/one of CCPHC's clinics.
• Immunizations received in Missouri that were reported by the healthcare provider to ShowMeVax.

Please do not walk in to the health department to request immunizations records. Completed records request can be picked up from the health department or delivered via mail, email, or fax.
