Please register no later than 2 weeks before the clinic takes place. We cannot accept registrations received after the deadline.

After completing the registration form, you should see a confirmation message. Then, a Clay County Public Health Center staff member will call you to schedule an appointment, within 2-3 business days after submission. Please look out for and respond to our calls so there are no issues on the day of the clinic.

Prepare for Clinic

On the day of their clinic, students should eat breakfast or lunch and be well hydrated.

The student should wear short sleeves or a tank top for easier access to the upper arm.

A parent or legal guardian must be with the student at all times during the clinic. *If a parent or legal guardian is unable to accompany the student, the parent or legal guardian may provide written consent for another adult to be with the student. The consent is included as part of the registration form.

Payment

Clay County Public Health Center accepts and will bill most major insurance plans including Medicaid. For students that DO have any type of insurance coverage, please provide this information on the registration form. Failure to do so may result in a delay of scheduling the student for requested immunizations.

Students without insurance, out-of-network insurance, or insurance that doesn’t pay for vaccines can also receive the vaccine at no cost during school immunization clinics.
